Rates
Current Birchwood Child Care rates for each age range and for special events or Saturday care. |
Contract
Agreement between parents and Birchwood Child Care. Promissary and service level agreement. MUST have this form for any new child enrolling in Birchwood Child Care. |
Child Information Form
Parental and Child general information such as contact information, addresses, phone numbers, work, acceptable persons to pick up your child, etc. |

Child Profile Form
Give us an overview of child habits including eating, sleeping, or other issues concerning the child.

Emergency Contact Form
Supply us with the contact information for you and alternate contacts in case of emergency. |
Photo Permission Form
Agreement to allow photos of your child to be used in the Birchwood Child Care website and other listed media. |
Field Trip Permission Form
A per-trip authorization form that MUST be submitted to allow your child to attend the specific field trip. You will need one of these filled out for each trip for each child. |
Off-Site Permission Form (walks/hiking)
You need to have this form filled out for your child to be able to leave the building. Generic form we keep on file for any trips around the property. |
